First Visit

Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Occasionally, treatment can be done the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.

Please assist us by providing the following information at the time of your consultation:

• Your referral slip from your General Dentist and any X-rays if applicable.
• A list of medications you are presently taking.
• Your Driver’s license or other form of picture ID.
• Your dental and medical insurance cards. This will allow us to help you process any claims and minimize out of pocket cost.

X-Rays

Our office will typically take panoramic X-rays during your first visit. 

IMPORTANT: All patients under the age of 18 must be accompanied by a parent or legal guardian at the consultation visit.
